1) Prescription from Doctor or Chiropractic Physician
A prescription for massage therapy must include the following information:
- The referring physician's full contact information, including their National Provider Identification (NPI) number
- The diagnosis code(s)
- The number of sessions prescribed
- CPT codes (97124, 97140) that indicate massage and manual therapy
2) MVA Intake Form
- Download and email me a completed copy of the MVA Intake form
- Insurance company’s name, address and phone number
- Claim number and the date of incident
- Claim adjuster's name, telephone number and email address
